Self Identification Form

This is the initial form that must be submitted to SAS to request accommodations.
At the time of registration, you must present one of the following types of documentation.

Textbook Request Form

 This is how you request your textbooks in digital format if listed as an accommodations.

Residence Special Accommodation Request Form

 This is where you can request disability-related living accommodation in residence.

Request for Final Exam Accommodations - Late Booking Form

 This is required if you missed the deadline for final exam bookings and would like to request an accommodated test.
