Blue Sky Family Health Team (BSFHT) is seeking a Registered Social Worker (RSW) to join our interdisciplinary primary care team. This role is ideal for clinicians who enjoy structured, goal-oriented counselling and working collaboratively within a team-based medical environment.
The RSW provides brief, solution-focused therapy and psychosocial support to patients experiencing mental health and life challenges, with an emphasis on practical strategies, coping skills, and coordinated care.

What to expect in this role:

  • Provide individual and couples counselling and psychotherapy using a short-term, goal-focused model (typically 4–6 sessions per patient)
  • Conduct 1-hour sessions (initial assessments are longer)
  • See up to 5 patients per day (actual daily volume can be lower due to no-shows and scheduling flow)
  • Maintain an active caseload of approximately 40 patients
  • Work within a two-week booking window to support timely access
  • Complete psychosocial assessments and develop client-centred care plans
  • Support patients with anxiety, depression, trauma, and life stressors using practical, skills-based approaches
  • No ongoing crisis management or after-hours/on-call responsibilities
  • Initial appointments are coordinated by administrative staff; ongoing care is managed by the RSW
  • Collaborate closely with physicians and team members using a shared electronic medical record (EMR)

Practice environment:

  • Interdisciplinary primary care model with close collaboration with affiliate physicians
  • On-site team includes, social work, allied health professionals, administration, and management
  • Integrated care supported through a shared EMR and coordinated communication pathways
  • Structured access model with defined referral triage and time-limited episodes of care and discharge planning

Qualifications:

  • Master of Social Work (MSW)
  • Registration (or eligibility) with the Ontario College of Social Workers and Social Service Workers
  • Strong assessment, counselling, and communication skills
  • Experience (including practicum) in mental health or counselling
  • Experience with EMRs is an asset
  • Bilingualism (English/French) is considered an asset.
