With Registration:

1. An official transcript showing that an approved degree has been conferred;
   
2. Legal proof of change of name (if not provided upon application) if different than that on the birth certificate (e.g. marriage certificate);
   
3. Criminal Reference Check required for practicum.

Note: 

In order to receive an Ontario Certificate of Qualification, graduates will be required to provide the College of Teachers with proof of Canadian citizenship or status as a permanent resident under the Immigration Act (Canada) as well as a criminal record check.
