Self Identification Form

In order to better meet the needs of students with disabilities at Nipissing University, this early identification and information form is made available for you to facilitate early communication with the Disability Services Office.

This information is collected according to the following principles:

Participant identification is voluntary.
If you choose to identify yourself, all information is CONFIDENTIAL to Counselling and Disability Services and cannot be released without your permission.
Identification will not affect your admission status.

Please complete the attached form and forward it to Daralynn D'Angelo (daralynd@nipissingu.ca), Mike Walker(mikew@nipissingu.ca), or fax to 705-495-2850.

The purpose of requesting this information is to assist the Disability Services Office to make early accommodations or arrange special services if requested by a student with a disability.

Returning Student Form

For those students who have previously registered with Disability Services, and will be registering again in the new school year, please fill out the form below and return it to the Disability Services Office to expedite your registration process.

Virtual Intake Forms
for Online or Distance Education Students

If you are an off-campus student with a disability, registered in Nipissing's online or distance education courses through the Centre for Flexible Teaching and Learning (CFTL), you can register with the Disability Services Office (DSO) without visiting the North Bay campus.
Once you have Self-Identified to the DSO and provided current disability-related documentation to your learning strategist, you can register for our services by completing the following two forms and emailing them to your learning strategist from your Nipissing email account.

Once you have registered with the DSO, you must contact your learning strategist, the CFTL and your instructors at the beginning of each new term. For more information about these services click here for the Student Handbook.
