Two options of the form:
Supplementary Materials Request Form - fillable pdf - Note: To send this form to FASS electronically, you must fill it out, save it to your computer, then attach it to your email.
Supplementary Materials Request Forms - pdf- Print the form, complete it, scan it, then attach it to your email.
Test must be designed so that answers are either ‘true’ or ‘false’ (a or b) or ‘multiple choice’ with up to five selections (a, b, c, d or e).
The scanning program only recognizes one correct answer per question and is not able to deduct marks for wrong answers.
When designing your test questions please keep in mind that the scanning program can only work with questions valued at 1 mark each.
What the Students Should Know When Writing the Test
Students must use a pencil, black or blue ink. Red, green and purple inks are not recognized by the scanner.
Students must fill in their last, then first name in the bubble section.
Circles should be filled completely and any corrections erased completely.
The bar code must be left undisturbed.
