Associate Teacher Information - Forms

 
Payment and Registration Form
 
Before an Associate Teacher is able to start completing an online report, they must complete the Payment Registration form located in WebAdvisor, found here.
 
·         To get to the Payment Registration form, click on the “Teachers or Masters Students” tab within WebAdvisor.
·         On the next page under the “Associate Teacher” heading, select “Payment Registration”.
·         Complete the form by entering in all of the fields required.
·         An OCT number must be given.  To look up an OCT number, click “Find my OCT number” to be directed to a web page to search by name
·         The form is completed by selecting “Submit”.

Submission of Practicum Evaluations
Preferably evaluations for Teacher Candidates should be completed on-line.  If an online evaluation is not submitted, a copy of the completed evaluation should be submitted to the Practicum Office within five days of each completed session.
