Associate Teacher Information - Forms

Payment Form

Before an Associate Teacher is able to start completing an online report, an Associate Teacher must complete the payment form located here.

( “Silverlight” may need to be installed by clicking RUN when prompted)

  • Complete the form by entering in all of the fields required.
  • An OCT number must be given. 

*To look up an OCT number click “Find OCT#” where the OCT website will be displayed.  “Find A Teacher” is located on the website which brings up a field to search by name.

  • On this form a password needs to be created - please ensure this can be easily remembered as this password is needed to access the reporting site.
  • The form is complete by selecting “submit”.  A confirmation will be sent to the email address provided on the payment form.  This verifies the information has been submitted.   The information will then be matched by the Practicum Office (this usually takes up to 2 business days)
  • Another email will be sent when the login is ready to be used on the reporting site.

This will bring up a prompt for a username and password needed to login to the account.  Please remember the login is the OCT # and the password is a unique one created on the payment form.

 

  • The “User Name” that is required to login is the Associate Teacher’s OCT number.
  • The “Password” is the unique password created by the Associate Teacher on the Payment Form earlier.

(if this password is lost or forgotten please contact the Practicum Office)

  • Once this information is added, select “Log In”

Once properly logged into the reporting site the following screen will appear:

The information should be automatically populated with the Student Name and which Term report you can complete.

  • Beside the student’s name it shows the word “Placement” and in brackets the current report being displayed.
  • Depending on the report to be completed (Term 1 or Final), the drop down menu should be used to select the correct report.  The report highlighted in blue is the one selected.

Once the proper report appears the evaluation mark and comments can be entered (see images below)

When completing this part of the report please keep in mind the following:

  • Spell Check is available when completing reports.
  • Include in the comments- strengths and areas to improve upon.
  • There is a maximum of 1200 characters of text for each comment box.  It is suggested that Associate Teachers write in paragraphs, instead of bullets, in order to make use of the limited space.
  • The report does not need to be completed in one session.  The report site allows the user to save and retrieve the report for completion in a later session.
  • At the bottom of the report screen there are two buttons (Submit to PT Office, and Preview/Print) – (see image below)
  • “PREVIEW/PRINT” allows for Associate Teachers to see if the comments made are visible in the space provided and also to print a copy for their own records.  Please ensure all comments can be seen before submitting report to the Practicum Office.
  • Upon completion of the report, the Associate Teacher can save then select SUBMIT TO PT OFFICE.

  • Once the report is submitted to the Practicum Office, the user will still have access to the report, but on a ‘view only’ basis.
  • Once the Practicum Office has received the report, it is e-mailed to the student, and Faculty Advisor.

The process is complete!

Submission of Practicum Evaluations

Preferably evaluations for Teacher Candidates should be completed on-line.  If an online evaluation is not submitted, a copy of the completed evaluation should be submitted to the Practicum Office within five days of each completed session.
