The donation form would be used to receive your donation of documents or photographs in the original format or as digital copies. It would give full rights to the material donated to Nipissing University, our parent body.
The consent form included here meets the requirements of government funded research. Even if you are not funded, it would still be a good idea to get a consent form signed. You then know exactly what the person has agreed to if you later decide to publish your project. If the person agrees you can then donate the tape of the interview to the Institute or your local museum for use by researchers. If the final donation of rights section is signed the tape can be donated to the Institute and we will know that we can publish the full transcript or place the video on the web. The form should always be kept with the actual tape. A second copy of tapes should also be made for safe-keeping.
The release form would be filled in to give you permission to publish the images or documents which belong to someone else, in a very specific manner, like a web site or a book.
